Rehabilitation as prescribed by a referring practitioner; establishes goals of physical therapy and re-assesses patient’s progress at regular, timely intervals, supporting findings with written documentation. Maintains a high level of professional expertise. Plans, directs and supervises all technical aspects of the department with regard to services.

Provides speech therapy services to patients in the home with speech, language or swallowing deficits. Assumes primary responsibility for coordinating home health care for patients if nursing or physical therapy is not primary. Makes initial/ongoing assessment of the patient=s needs, identifying changes and trends in various physical and emotional parameters

Description
 
Administers treatment to patient in accordance with the physician’s referral.  Organizes and conducts speech therapy program in the home health setting to facilitate rehabilitation to the speech and language involved patient.
Qualifications
Education:

Successful completion of a Masters level accredited speech pathology curriculum
