Complete Description

Conducts assessment of the patient’s clinical, social, functional and continuing care needs. Will have responsibility for assessing patients on assigned units within 48 hours of admission which includes review of the treatment plan, determination of the appropriate level of care, assessment of discharge needs and initiation of the discharge planning process.  Conducts daily rounds on assigned unit to ensure that the multidisciplinary plan of care is consistent with the patient’s clinical course, continuing care needs and covered services.  Continually reassesses the plan of care and discharge needs of the patient and collaborates with the members of the multidisciplinary team to modify the plans as needed based on the patient’s changing needs.  Will have knowledge of behavioral responses to illnesses and other areas (community resources, payer requirements) to facilitate the patient’s movement along the health care continuum.

Registered nurse/Telemetry 

Graduate of an accredited school of nursing and current RN licensure in the State of Florida. Attends and participates in in-service training and various educational programs for professional development, and keeping abreast of developments in the field. One-year previous experience in Critical Care Nursing within the past five years or previous Med/Surg experience with completion of a Critical Care IMCU internship. Ability to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’’s requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit/area/department policies and procedures. This position exists to assume principle responsibility under general supervision, for the total care, assessment, implementation, and evaluation of each patient assigned to the Patient Care Unit either directly and/or through leadership of personnel. He/she develops and/or revises a plan of care. Initiates healthcare training and education to the patient and/or patient’’s family to facilitate recovery. Utilizes principles of teaching and learning to assist patients, staff, and self, in the identification of the need for new knowledge and skills.
