Clinical Director, Critical Care

Description:

In a 250 bed community hospital setting, this Clinical Director would provide the leadership for a 38-bed combined Med/Surg & Cardiology Critical Care unit.  There are 95 FTEs on this unit.  An Intensivist is onsite 24 hours a day.  The unit admits a mix of medical, cardiac, and surgical patients.  This is a very high tech, computerized facility, with most information available on-line and at the bedside.  The position will report to the Senior Director of Acute Care & Critical Care Services who reports to the CNE & VP, Patient Care.  This organization has a shared governance model and is working towards Magnet Status.

Qualifications:

BSN, Masters degree and 5 years of relevant critical care experience is required, as well as 3 years prior management experience.  Cardiology experience a plus. CCRN required.

Description

Qualifications

Associate degree in Health Information Management required; Bachelor’s degree in HIM preferred.  Minimum of three years at the Director level or minimum of five years at the Assistant Director level.  Knowledge and experience in health information privacy laws, access, release of information and release control technologies.  Demonstrated organization, facilitation, communication and presentation skills.  Registered Health Information Technician or Registered Health Information Administrator certification required.  Certified Coding Specialist required.

ULTRASONAGRAPHER

Job Summary:

Prepare patients for and perform obstetrical ultrasound exams in accordance with physician’s orders. Prepare films and exam/results in a timely, accurate manner to facilitate diagnosis and ensure quality patient care. This position must demonstrate a commitment of quality service to our patients, the public and our co-workers.

Perform fetal monitoring (fetal well-being) tests on indicated patients.
Following the established AIUM and ACOG guidelines for ultrasound, perform and document the appropriate images for the study indicated in order to provide the physician with comprehensive views to give an accurate interpretive report.
Facilitate lines of communication between the patient and the physician.
Ultrasonographer is responsible for the care of the ultrasound equipment. Works with the Biomedical Services Department and other technical support personnel to insure equipment functions properly.
Maintain continuing medical education credits by attending ultrasound seminars each year. Maintain an active status in American Registry of Diagnostic Medical Society and the Central Virginia Diagnostic Medical Society. Stay current with OB/GYUN literature by reading professional journals and books.
Order medical supplies from appropriate vendors in order to maintain a sufficient inventory of medical supplies.
Provide consultation to Registrar in scheduling patient and accommodating non-routine referrals.
Communicate effectively with patients and their families in difficult and anxious settings and provide appropriate patient care.
Assume other duties as assigned.

Qualifications:
AART registered in Radiologic Technology or RDMS-Obstetrics registered.
Skill in operating GE Ultrasound Machines
Skill in identifying and recording ultrasound images required for a particular study to insure a complete and thorough exam.
In-depth knowledge of physiology and anatomy, both female and fetal.
Experience in the area of OB/GYN ultrasound with a minimum of 2 years experience in a high risk OB/GYN environment

We are currently seeking a full-time experienced endoscopy registered nurse licensed or eligible for licensure in the state of VA, with current hospital-based endoscopy and conscious sedation experience. The endoscopy department at John Randolph Medical Center is a 12-room operation with an average daily census of 10-15 patients. We are rebuilding our endoscopy department with new physician practices and seek an experienced nurse to join our growing team!

Requirements:
3-5 years of RN experience in a hospital endoscopy setting. Must have conscious sedation experience and ERCP experience. Must be currently certified by the American Heart Association in CPR and ACLS. Excellent customer service and communication skills required, and experience with electronic documentation system is highly preferred.

Endoscopy department operates Monday-Friday from 6:30am-3pm, with call coverage around the clock. This position does require rotation in the call coverage schedule to include second, third and holiday call. We do staff with RNs and experienced endo techs.

Case Management Manager/Virginia

Coordinates the Case Management Program.  Supervises and inservices staff case managers. Must maintain up to date knowledge of any Joint Commission or Medicare/Medicaid regulatory changes. Assists QRM Director in planning and implementing these changes. Responsible for planning case management meetings with a multidisciplinary team including physicians. Assists QRM Director in competency assessments and annual performance evaluations of case managers. Also must take a case management assignment which includes these duties: Serves as a case manager to monitor and coordinate a comprehensive program of admissions reviews, continued stays and discharge plans of the patient.  Processes appropriate data to monitor and guide acute care hospital coverage for patients.  Conducts medical record audits and evaluations.  Prepares statistical analysis in accordance with departmental, organizational, Joint Commission, and other licensure agencies requirements.  Initiates and directs case management of patients from preadmission through discharge for appropriate movement through the continuum of care while providing quality care advice to meet specific patient needs.  Monitors cases, develops and implements plans for appropriate documentation and length of stay compliance.
Qualifications

Education:  Graduate of accredited School of Nursing.  RN required.  Must have BSN or be enrolled in a degree program and actively pursuing a BSN.

Experience:  At least five years of clinical direct patient care nursing experience in an acute care hospital required. At least one year of nursing supervision experience preferred. Previous Nursing/Case Management/Utilization Review/Discharge Planning experience preferred.

Skills:    Current licensure by Virginia State Board of Nursing.  Ability to communicate effectively.  Knowledge of Basic Computer Skills.  Ability to work under high stress situations and maintain professional attitude.  Ability to accept responsibility.  Ability to develop and  implement policies and procedures related to case management. Essential Functions & Responsibilities :

Answers telephones courteously, professionally, and by the third ring. Screens and transfers telephone calls or takes messages as appropriate to facilitate effective Radiology communications.

Effectively interacts with students, consistently providing supervision and instruction. Is supportive of the MWH School of Radiologic Technology and the ultrasound cross-training program.

Monitors and maintains an adequate inventory of supplies and materials to ensure their continued availability and non-interruption of services.

Reports, records, and files patient and procedural data in order to accurately document department operations and facilitate timely accurate billing for services.

Participates in performance improvement activities for the Ultrasound Section utilizing the FOCUS PDCA (Plan, Do, Check, Act) approach to improvement.

Demonstrates the knowledge and ability to meet the age specific needs of the patient groups identified below under Qualifications.

Qualifications:
RDMS registered or registry-eligible in Ultrasonography. (If registry-eligible at time of hire, must pass registry examination within one year of hire date.) Preferably ARRT registered in Radiologic Technology.

Preferably AART registered in Radiologic Technology.

Registry in Abdomen/Small Parts, OB/GYN, Vascular, and Neurosonography preferred, must pass OB/GYN Registry within 6mo of hire.

Completion of accredited program in ultrasound or six months relevant experience.

Three years relevant experience preferred.

EXPERIENCE Required -Minimum of two years as a registered vascular tech and concentrated knowledge of vascular anatomy and physiology.

*Interprets exam results to quantitate the extent of patients disease by utilizing established diagnostic parameters.

*Initiates and documents patient teaching including family and/or significant others.  Uses appropriate patient education documentation modality.  Teaches patients about vascular disease to increase patient knowledge of disease process.  Refers questions regarding complications and treatment modalities to the patient’s physician.

*Pursues professional development to prepare for additional responsibilities and improve current job performance by participating in educational meetings, independent study and maintaining appropriate certification.

*Interfaces with the Department of Radiology by correlating non-invasive test results with contrast radiographs to maintain quality control and establish accuracy of report.                                                                 *Maintains Lab equipment to ensure optimal functioning by inspecting for breakdown, troubleshooting and communicating with appropriate repair persons.

*Assists with departmental CQI program, outcomes management projects and other hospital initiatives.

*Assists in Conducting a thorough and complete orientation of all incoming personnel, utilizing the department orientation plan and ensuring all aspects are completed during the orientation process

Essential Functions & Responsibilities:
 
Manage overall Department operations to ensure timely, effective case management of all patients with placement of the patient to the appropriate level of care.  Works collaboratively with physician leadership to develop and implement care management operations.
Develop and implement operational policies and procedures to ensure continuous, efficient operation and utilization of resources.  Analyze financial and cost data.
Design and monitor continuing education plan and programs for the Department associates including participation in Hospital in-service education to facilitate Departmental development and operation.
Direct and monitor the performance improvement activities of the Department operations to ensure timely, effective utilization of resources, effective utilization management, discharge planning and follow up of patients.
Develop and manage Department budgetary process to ensure cost effective operation.  Responsible for recruitment and retention of appropriate Associates.
Interface with Senior Management, Department Managers and Physicians to facilitate effective interdisciplinary planning,  for effective utilization of resources.  
Assist in facilitation of organizational change through effective collaboration and networking.
Demonstrate and foster open and effective communication with Associates on an ongoing basis in a confidential, nonjudgmental manner while providing an environment for open and willing listening to others ideas and opinions.  Provide timely counseling and ongoing feedback to Associates.
Conduct staff and team meetings on a routine basis to enhance team development and information sharing.
Demonstrate effective leadership through anticipating issues and formulating plans to address situations.  
Role model problem solving, decision making and collaboration.  Model health system values.
Provide appropriate reports and documents to effectively communicate operations of department.
Work closely with physicians, finance and managed care to maintain acceptable denial rates.
Responsible for coaching and facilitation of associate development through performance evaluation/peer evaluation. Empower staff to make decisions that lead to customer satisfaction.
Responsible for working with physicians and other Associates to develop, monitor and improve case management through appropriate utilization of pathways, protocols and standing orders.  Utilize benchmark data to improve indicators of cost, quality and service.
Demonstrate professionalism and positive attitude through positive verbal and nonverbal communication skills, consistent positive approach, on time and prepared for meetings, professional attire, smiling and an approach to problem solving that is in the order of interest for the System/Hospital/Department.  Support the customer service initiative and maintain an effective customer service recovery plan.
Maintain professional competency and education growth.

MAJOR FUNCTION:
Assess and administer appropriate physical therapy treatments in patient homes for a variety of diagnoses. Work with the health care planning team to provide for continuity and quality of care and appropriate planning for discharge.

QUALIFICATIONS AND COMPETENCE DETERMINATION:
I. Education: A degree in physical therapy (Bachelor’s, Master’s, or certificate), Graduate of an accredited program in physical therapy. Licensed physical therapy assistants will also be considered.
II. Licenses/Certifications: Current Virginia State Licensure (or eligibility) to practice physical therapy, Current CPR certification, Valid Virginia Drivers License
III. Experience: Two years experience as physical therapist is preferred, Experience in HomeCare with geriatric patients and/or in a rehabilitation setting is desirable.

ESSENTIAL JOB DUTIES AND COMPETENCIES:
1. Demonstrates competence in the provision of quality patient care: careful assessments; establishment and revision of appropriate goals and treatment plans; provision of patient and family education; and referral to other disciplines/agencies as appropriate.
2. Responsible for appropriate and timely documentation of evaluations and treatments in compliance with JCAHO, Medicare, Medicaid, third party payors, and department standards.
3. Responsible for communicating effectively orally and in writing with physicians, ancillary staff, patients, families, and caretakers regarding patient status, progress, response to treatment, goals, and plan of action to ensure quality care.
4. Responsible for the appropriate direction of and delegation to Physical Therapist Assistants, Home Health Aides, and students regarding patient care according to state law and department standards.

6. Assists in the development and implementation of programs and protocols.
7. Participates in team meetings, patient care coordination with other home care disciplines, public relations activities, health screens, educational programs, and clinical/staff meetings.
8. Assists in the orientation of new Physical Therapists and Licensed Physical Therapy Assistants.
9. Responsible for scheduling home visits, submitting schedule, and contacting patient/family and office for changes in pre-arranged visits.
10. Rotates call, weekend and holiday coverage with other physical therapists.

Demonstrates competent equipment operation, specific to area of service, during initial orientation, as new equipment is acquired, and annually if designated as a department competency.
