Description

24-hour accountability for the operations and oversight of the Health Information Management (HIM) department and HIPAA Privacy for  Regional Medical Center.  Plans and manages the preparation and maintenance of health information management systems for the hospital to ensure accurate and comprehensive patient care data for patient evaluation, internal quality audits and historical purposes.  Demonstrates the ability to meet the standards of regulatory agencies including Joint Commission, state licensure and others.  Demonstrates fiscal responsibility in management of departmental budgets.

The Privacy Officer oversees all ongoing activities related to the development, implementation, maintenance of and adherence to the organizations policies and procedures covering the privacy of, and access to, patient health information in compliance with federal and state laws and the healthcare organization’s information privacy practices.
Qualifications

Associate degree in Health Information Management required; Bachelor’s degree in HIM preferred.  Minimum of three years at the Director level or minimum of five years at the Assistant Director level.  Knowledge and experience in health information privacy laws, access, release of information and release control technologies.  Demonstrated organization, facilitation, communication and presentation skills.  Registered Health Information Technician or Registered Health Information Administrator certification required.  Certified Coding Specialist required.

We are currently seeking a RN Clinical Coordinator of Endoscopy to manage our endo department! With the addition of new physician practices, we are looking to rebuild our department to support increasing volume and growth. We currently have a need for a full-time experienced endoscopy RN Clinical Coordinator (must have conscious sedation and ERCP experience) with recent hospital-based endo experience and leadership experience. The department is a 12-room operation and the hours are Monday-Friday 6:30am-3pm, with rotating call coverage for second, third and weekend/holiday call. Coordinates the Case Management Program.  Supervises and inservices staff case managers. Must maintain up to date knowledge of any Joint Commission or Medicare/Medicaid regulatory changes. Qualifications

Education:  Graduate of accredited School of Nursing.  RN required.  Must have BSN or be enrolled in a degree program and actively pursuing a BSN.

Experience:  At least five years of clinical direct patient care nursing experience in an acute care hospital required. At least one year of nursing supervision experience preferred. Previous Nursing/Case Management/Utilization Review/Discharge Planning experience preferred.

Skills:    Current licensure by Virginia State Board of Nursing.  Ability to communicate effectively.  Knowledge of Basic Computer Skills.  Ability to work under high stress situations and maintain professional attitude.  Ability to accept responsibility.  Ability to develop and  implement policies and procedures related to case management. This position is accountable to prepare patients and perform routine ultrasound procedures in accordance with physicians’ orders and the procedure manual. The Ultrasonographer produces high quality images and prepares/presents them with appropriate comparison studies and other exam results or pertinent information to the Radiologist for timely interpretation. Essential Functions & Responsibilities :
Prepares patients for ultrasound examinations by confirming physicians’ orders, verifying patients’ identities, and obtaining adequate medical histories to ensure the appropriateness of examinations and facilitate timely accurate diagnosis of the patient.

Performs ultrasound examinations/procedures on all patient age groups and maintains clinical competency as evidenced by completion of an annual Competency Based Orientation Tool for Clinical Associates in Ultrasound. Adheres to departmental policies, procedures, standards, and protocols to ensure effective departmental operations and quality patient care. Demonstrates a high degree of knowledge and understanding of ultrasound services/activities.

Maintains a safe environment for patients, personnel, and visitors by requiring and supporting the establishment, maintenance, and education of an effective safety management program. Disinfects equipment and ultrasound probes according to hospital standards and adheres to standard precautions at all times.

Greets all patients, visitors, family members, physicians, and Associates in a courteous and professional manner. Addresses customers’ needs efficiently, effectively, and confidentially.

Answers telephones courteously, professionally, and by the third ring. Screens and transfers telephone calls or takes messages as appropriate to facilitate effective Radiology communications.

Effectively interacts with students, consistently providing supervision and instruction. Is supportive of the MWH School of Radiologic Technology and the ultrasound cross-training program.

Monitors and maintains an adequate inventory of supplies and materials to ensure their continued availability and non-interruption of services.

Reports, records, and files patient and procedural data in order to accurately document department operations and facilitate timely accurate billing for services.

Participates in performance improvement activities for the Ultrasound Section utilizing the FOCUS PDCA (Plan, Do, Check, Act) approach to improvement.

Demonstrates the knowledge and ability to meet the age specific needs of the patient groups identified below under Qualifications.

Qualifications:
RDMS registered or registry-eligible in Ultrasonography. (If registry-eligible at time of hire, must pass registry examination within one year of hire date.) Preferably ARRT registered in Radiologic Technology.

Preferably AART registered in Radiologic Technology.

Registry in Abdomen/Small Parts, OB/GYN, Vascular, and Neurosonography preferred, must pass OB/GYN Registry within 6mo of hire.

Completion of accredited program in ultrasound or six months relevant experience.

Three years relevant experience preferred.

The Clinical Nurse Specialist (CNS) is a registered professional nurse who demonstrates expertise in a specific area of clinical nursing that has been further developed through Masters preparation in Nursing. Using principles of case management, the CNS focuses on improvement of processes, treatments, plans of care and other related interventions and makes recommendations to improve the delivery of high quality care to all patient populations. The CNS functions as a clinical resource, providing clinical, professional, educational and quality improvement consultation to our Associates in pursuit of improved care delivery and improved patient outcomes. The scope of practice encompasses the use of the nursing process in: practice, education, consultation, research and administration. The CNS must also serve as a resource for hospitalized patients admitted to medical/surgical and other areas due to other complications.

Qualifications:

RN license in state of Virginia.
Masters in Nursing and advanced knowledge of Perinatal/Maternal/Neonatal/Pediatric nursing theory and practice required.
Professional certification required by the Virginia State Board of Nursing.
Minimum of five (5) years experience in formal/informal settings preferred: knowledge of adult learning principles required.
Skill in the use of specialized equipment used in respective area of practice.
Familiarity with philosophy, organizational structure and policies and procedures of the Perinatal/Pediatric Departments highly desirable.
Exceptional interpersonal skills: communication, collaboration and conflict resolution. Knowledge of systems and change process.
Demonstrated leadership skills, creativity, self direction and organization required.
Demonstrates sensitivity to cost containment measures and creates an atmosphere which encourages this practice by conserving hospital supplies, equipment and human resources.

MAJOR FUNCTION:
Assess and administer appropriate physical therapy treatments in patient homes for a variety of diagnoses. Work with the health care planning team to provide for continuity and quality of care and appropriate planning for discharge.

QUALIFICATIONS AND COMPETENCE DETERMINATION:
I. Education: A degree in physical therapy (Bachelor’s, Master’s, or certificate), Graduate of an accredited program in physical therapy. Licensed physical therapy assistants will also be considered.
II. Licenses/Certifications: Current Virginia State Licensure (or eligibility) to practice physical therapy, Current CPR certification, Valid Virginia Drivers License
III. Experience: Two years experience as physical therapist is preferred, Experience in HomeCare with geriatric patients and/or in a rehabilitation setting is desirable.

ESSENTIAL JOB DUTIES AND COMPETENCIES:
1. Demonstrates competence in the provision of quality patient care: careful assessments; establishment and revision of appropriate goals and treatment plans; provision of patient and family education; and referral to other disciplines/agencies as appropriate.
2. Responsible for appropriate and timely documentation of evaluations and treatments in compliance with JCAHO, Medicare, Medicaid, third party payors, and department standards.
3. Responsible for communicating effectively orally and in writing with physicians, ancillary staff, patients, families, and caretakers regarding patient status, progress, response to treatment, goals, and plan of action to ensure quality care.
4. Responsible for the appropriate direction of and delegation to Physical Therapist Assistants, Home Health Aides, and students regarding patient care according to state law and department standards.
5. Demonstrates competence in the provision of inservice and one-on-one training and consultation for hospital and department staff.
6. Assists in the development and implementation of programs and protocols.
7. Participates in team meetings, patient care coordination with other home care disciplines, public relations activities, health screens, educational programs, and clinical/staff meetings.
8. Assists in the orientation of new Physical Therapists and Licensed Physical Therapy Assistants.
9. Responsible for scheduling home visits, submitting schedule, and contacting patient/family and office for changes in pre-arranged visits.
10. Rotates call, weekend and holiday coverage with other physical therapists.

EQUIPMENT OPERATION:
Demonstrates competent equipment operation, specific to area of service, during initial orientation, as new equipment is acquired, and annually if designated as a department competency. Equipment includes: Wheelchair, TENS units, Portable Ultrasound, CPM Unit, Hoyer lift, EMS Unit, Basic office equipment, walker
