 Our Corporate Office in King of Prussia is looking for a Clinical Nurse Specialist.  The Clinical Nurse Specialist is a registered nurse, who has graduate level nursing preparation at the master’s or doctoral level. This includes the five domains of CNS practice-Clinical Practice, Leadership, Education, Research and Consultant.  He/she is a clinical expert in evidence-based nursing practice within a specialty area. Clinical Nurse Specialist can practice autonomously and integrate knowledge of disease and medical treatments into the assessment, diagnosis, and treatment of patients’ illnesses. Clinical Nurse Specialist advances the practice of nursing to achieve quality and cost-effective patient outcomes as well as provide leadership to multidisciplinary groups in designing and implementing innovative alternative solutions to address system problems and/or patient care issues.  In addition to providing direct patient care, a Clinical Nurse Specialist influences care outcomes by providing expert consultation for nursing staff, mentoring nurses to acquire new skills and by facilitating the implementation of improvements in health care delivery systems.

Education:Professional Certification and Masters required.  Doctorate preferred

Licensure:  Current license or permit to practice professional nursing in the state of PA.  License to practice as a CNS based on state regulations.

Professional Knowledge:  Certified as a CNS and certified in area of specialization as applicable.  Demonstrated advanced knowledge of nursing clinical practice and management of complex patient disease processes.  Extensive knowledge of principles and practices relevant to nursing and patient care activities.  Highly developed written and verbal communication skills. Must complete and maintain BLS.  ACLS, NRP and PALS certification as applicable to area of specialization.  Must complete and maintain instructor status for at least one of these certifications.

Experience:  Five or more years of clinical nursing experience in an acute care setting.

Interpersonal Skills:  Interpersonal skills to foster a collaborative environment.  Ability to plan, direct, and implement change.  Demonstrated leadership abilities that contribute to a positive work environment.  Promotes mutual respect and professionalism in working relationships.

 Summary:
Directs administration of  Clinic within authority of governing board by performing the following duties personally or through subordinate supervisors:
Essential Duties and Responsibilities include the following.  Other duties may be assigned. 

- Competent in requirements per state and federal regulation for position.

- Implement and ensure adherence to  policies and procedures as well as federal and state requirements

- Evaluate patient levels to determine appropriate staffing requirements for clinic; directs hiring, training, supervision and development of personnel

- Evaluate patient levels and determine ability to accept new patients based   criteria; handle new patient admission intake and continuing case management

- Report timely all required records, reports and data concerning staff, patients, and operational affairs of facility to corporate, appropriate agency, or governing body

- Maintain clinic compliance with federal, state, and local laws/regulations by receiving continuing certification from all statutory and regulatory agencies; communicate to corporate and executive management information and correspondence from regulatory agencies

- Reviews and evaluates case management of patients and ensures that care is effective and professional and oversees patient assessments.

- Ensures skilled care is provided in accordance with established patient plans. Work to enhance patient comfort and emotional well being.

- Assume ultimate responsibility of patient medical records. Unit Administrator is designated Information Security Supervisor

- Participate in meetings involving Governing Body, Vice President and staff; conduct staff meetings, in-service education meetings, and team patient care planning meetings as needed and required.

- Continual compliance of Quality Assurance Program; maintain safe and sanitary environment; maintain and upkeep all equipment; maintain inventory and supplies necessary

- Conduct monthly review of Committee reports including but not limited to, Quality Assurance and Safety Committees and Patient Care Meetings

- Serve on clinics Governing Body; local liaison to End Stage Renal Disease (ESRD) Network

- Establish and maintain positive relationship with local hospitals, agencies, and community by participating in outreach and marketing activities, which may include off-site meetings and promotional events.

- Administers fiscal operations such as budget planning and accounting for clinic
Education/Experience:
Graduate of an accredited nursing degree program; prefer bachelor’s degree (B. A.) from four-year College or university in related field with equivalent experience

Minimum three years Nephrology experience

Certificates and Licenses:
Current Registered Nurse (RN) License in state of practice; strongly preferred CNN or CDN, or current License Practical Nurse (LPN) in state of practice.

Current cardiopulmonary resuscitation (CPR) certification

- Conduct psychosocial assessment of each patient upon admission.

- Work in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further understanding of significant social and emotional factors underlying patient’s health problem.

- Participate in team review of each patient to develop CIPA & POC’s, to review patient progress, and to recommend changes in treatment based on the patient’s current psychosocial needs

- Conduct a financial evaluation of each patient prior to admission; discuss financial aspects of dialysis with patient and/or family prior to the first treatment; including transient patients.

- Verify current insurance benefits related to in-patient, outpatient, and professional services.  Record benefit information and obtain copies of proof of insurance and assignment of benefits for facility record.  Submit all required information and copies to the corporate on or prior to day of admission.

- Assist patients in completing applications for additional financial coverage, such as Medicaid, and state or special programs; provide follow-up on determinations of eligibility; and document proof of continuing eligibility.

- Maintain current information on all financial assistance programs related to dialysis, including eligibility criteria and changes in benefits and covered services.  Work directly with agencies and provide education and assistance to patients in dealing with agencies.
- Help patient and family through individual or group conferences to understand, accept, and follow medical recommendations associated with End Stage Renal Disease (ESRD)

- Maintain a thorough knowledge of community social agencies and other resources to assist patients and their families to utilize them effectively.

- Prepare initial patient assessment, patient histories, service plans, and monthly progress reports in medical records to reflect all social services provided.  (Include, but not limited to, identification of current problems, current social work goals, actions plans and an evaluation of patient progress)

- Participate in planning for improving health services by interpreting social factors pertinent to development of program

- Assist other personnel through education and direction to support the social services program by learning to effectively deal with psychosocial problems as they arise.

- Maintain continuing education including participation in professional organizations related to End Stage Renal Disease (ESRD) patient care management.

- Participate in staff meetings or in-service education meetings when requested by the Clinic Administrator.

- Social Work Consultant services are obtained through contractual agreement based on an hourly fee, not to exceed a specified number of hours per patient per month for each facility.

Education/Experience:
Master’s degree in Social Work (M.S.W) or equivalent; with specialization in clinical practice from a graduate school accredited by the Council of Social Work Education; Must have minimum of one year in a dialysis unit  or transplantation  program with two years experience as a social worker.
Certificates and Licenses:
LCSW as required by state regulations.
