In addition, the Care Management Coordinator will coordinate care needs with the patient’s primary care and specialty providers and other disciplines. Services may be performed in person at provider sites, in the patient’s home, at community sites or telephonically. Assessments and interventions are recorded in the electronic health record and other specified. Bachelors degree required. Minimum five years of healthcare experience required. Two years of Diabetes educator/case management in an inpatient, HMO, Insurance or other outpatient setting preferred. Bilingual English and Spanish preferred. New York State Clinical licensure required: RN (Registered Nurse), or LMSW (Licensed Masters Social Worker). Experience with electronic medical records required. Diabetes Educator certification preferred.
Computer proficiency in Word, Excel required. Excellent interpersonal skills. Ability to problem solve and prioritize. Flexibility and ability to work in fast-paced, changing environment

Requirements:
•Has or can obtain any necessary License in-state of employment
•Minimum of three years nursing experience required.
•Prior experience in specialty or skill-set associated with position required.
• Able to demonstrate excellent communication skills and ability to work collaboratively with all members of the health care team.
•Requires physical stamina and emotional stability.
