 POSITION SUMMARY
This position assists the supervisor in organizing and directing the activities of the infection control program for all departments throughout the facility including surveillance, infection prevention and control measures, and education for employees. This position provides clinical data for monitoring and improving clinical performance; includes peer review, and physician profiles and to meet accrediting and regulatory standards. This position also supports specific hospital wide quality and patient safety programs. Work processed by the incumbent is confidential and protected from discovery by subpoena pursuant to ARS 36:445 and 36:441 or CRS 12-36.5 through 104.

ESSENTIAL FUNCTIONS
1. Conducts focused surveillance for healthcare-associated infections (HAIs), detects and records HAIs on a systematic and current basis, analyzes HAIs and prepares reports for the Infection Control Committee of the facility. Reports communicable diseases as required by regulation. Makes recommendations when problems are identified. Interprets department policies and procedures in conjunction with the supervisor.

2. Interacts with physicians, nurses, department managers, supervisors, and other professional and non-professional staff members as a consultant. Enters data into computer, prepares reports, conducts analytical studies and summarizes surveillance data. Initiates epidemiological investigations of significant clusters of infections and single cases of unusual infection.

3. Assists with continuing infection control education and in service programs to departments, including orientation for new employees. Reviews and assists with evaluation of hospital policies, procedures and guidelines as they relate to infection control. Assists in writing and revising infection control policies and procedures.

4. Makes recommendations regarding construction, renovation, environmental rounds, and assures compliance with national and/or professional standards.

5. Abstracts clinical data and develops data collection tools for population specific or focused review while maintaining clinical knowledge in practice guidelines and standards. Maintains data integrity through knowledge of department, facility wide and regulatory indicators, system initiatives, benchmarking standards, review of data for problem identification and significance and follow up as appropriate.

6. Analyzes patient care and practice patterns within nursing, ancillary and medical staff and identifies issues with opportunities for improvement. Identifies patient safety issues, events, trends and deviations from standard of care.

7. Utilizes electronic data systems both clinical and financial, develops/modifies queries, and assists with creating reports needed for improvement activities and reappointment of medical staff. Assists and supports peer review activities, when appropriate.

8. Maintains expertise and acts as a resource in clinical data base requirements such as: CMS core measures, JCAHO ORYX, STS, ACC and other facility or system clinical databases. Acts as a resource for data entry staff and new personnel.

Qualifications:

MINIMUM QUALIFICATIONS

Requires a current Registered Nurse (R.N.) license with BSN preferred.

Must have a minimum of 3 years clinical experience in a hospital setting or disease surveillance experience in a public health setting.

Must have good communication skills. Must have strong organization skills and be able to work independently.

PREFERRED QUALIFICATIONS
Association for Practitioners in Infection Control (APIC) training course or equivalent desired. Certification in Infection Control (CIC) is preferred. Computer skills, research and basic statistical knowledge is preferred.

Additional related education and/or experience preferred.

JOB SUMMARY:

The registered nurse is responsible for delivery of nursing care to individuals based on comprehensive assessment, analysis, collaboration with integrated team members, and educational needs of the patient.  The registered nurse is accountable for on-going evaluation and documentation of all components of care delivered to customers of Banner Churchill Community Hospital.

ESSENTIAL FUNCTIONS:

1.  Provider - Coordinates care of the patient and family using a multidisciplinary holistic approach that includes assessing the physical, psychological, social, spiritual, educational, developmental, and cultural and discharge planning needs by assessing patient’s physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.

    * Formulates a plan of care, including the discharge plan, utilizing assessment data, patient, family and health team input.
    * Implements the plan of care through direct patient care.
    * Evaluates the patient’s and family’s response to care.   
    * Documents assessment, planning, implementation and evaluation of care.
    * Follows standard precautions in the provision of patient care and implements other infection control measures as necessary.
    * Utilizes equipment according to manufacturer’s recommendations and hospital policy.

 2.  Teacher - Uses nursing theory and research to provide individual and family education.
    * Assess the patient’s and family’s preference for learning methodologies.
    * Implement the teaching about discharge on admission using the preferred method of learning.
    * Evaluate the patient’s and family’s response to the teaching.
    * Document the teaching and patient, family members understanding

3.  Advocate - Provides a climate in which individuals and families may act in their own interest.  RN’s intervene when patients are unable to act in their own interest.

    * Develop the goals for patient in conjunction with patient and family.
    * Maintain the plan of care developed with the patient and family and communicate with other health care providers the patient and family wishes/requests as the plan of care requires changes.
    * Evaluate the impact of the plan on the patient and family
    * Document the patient’s /family’s wishes to better communicate with all other health care providers.
    * Reports/removes unsafe equipment.

4.  Manager - Implements the plan of care through coordination, delegation and supervision of the activities of the multidisciplinary team working to achieve desired outcomes through implementation of individual and family care with effective use of time, resources, equipment and work systems.

    * Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel.
    * Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
    * Reports/documents unexpected occurrences involving patients and others as appropriate.
    * Follows hospital and unit specific emergency and disaster preparedness plan.

5.  Member of the Profession - As a member of the profession, a RN is accountable for the safety, ethical, legal and professional responsibilities related to all nursing functions.
 * Ensures care is provided on bases of physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirement.

6.  Employee must demonstrate age specific competencies for patient population(s) to which they are assigned.

In addition to the responsibilities described above, the role may include other responsibilities and duties as assigned on occasion, based upon Banner Churchill Community Hospital’s needs or requirements.

Follows safety standards in all aspects of performance of the above functions, reports/removes unsafe equipment and attends safety education sessions.

Counsels, educates and instructs patients and their families regarding their speech-language pathology treatment
Demonstrates competency in rehabilitation procedures
Maintains accurate patient records
Qualifications
Job qualifications include:

Masters Degree in Speech Language Pathology required.
Must be currently licensed by the state as a Speech Language Pathologist
Current certification as a BLS Healthcare Provider required.
Previous experience in an acute care hospital setting required.
Requires strong communication and interpersonal skills.
Must be able to work in a fast-paced environment.
