Director, Outpatient Care Management

is responsible for the design, implementation and management of a comprehensive Outpatient Care Management program that is accountable for the management of complex/catastrophic patients assigned to  Network Primary Care Physicians. The Director, is responsible for managing a staff of Registered Nurses who utilize the Case Management process to 1) assess the health care needs of the enrollee; 2) develop a comprehensive treatment plan complete with specific goals and objectives; 3) implement a treatment plan in collaboration with the PCP team and the other providers involved in the patient’s care; 4) negotiate and coordinate services for the patient; 5) monitor and evaluate the effectiveness of the plan in achieving the goals and objectives; and 6) change and modify the plan as needs and situations change. The Director is responsible for ensuring that staff is appropriately deployed throughout the System to support the Primary Care Practice. The Director works closely with all of the members of the Primary Care service line and ensures that OP CM services are available and responsive to the needs of the patients and physicians. The Director will determine the appropriate location of staff-within the PCP office or centrally located and deployed telephonically. The Director will develop policies, procedures, workflows and processes to ensure comprehensive Care Management services are available throughout the System. Adheres to the CMSA Standards of Practice for Case Management.

Demonstrates actions consistent with Expectations as duties are performed on a daily basis.

Requirements:  Master’s Degree with an active, unrestricted Louisiana RN license required; or BSN with 5 years recent experience in outpatient Case Management and certification as CCM may be substituted for Masters Degree. Minimum of three years recent experience in a manager or director role in a medical management environment Minimum five years experience in Care Management, including utilization or quality management; regulatory compliance. medical management or other related functions Certification as a Case Manager required; or ability to achieve within 1 years of hire. Under the direction of the Vice President, Accountable Care, the Director, Disease Management is responsible for creating innovative and effective chronic disease management programs that empower patients, facilitate the healthcare practitioner’s care plans, improve patient adherence to their treatment plan, and improve patient outcomes. This position is further responsible for creating the metrics and other Quality Improvement indicators to monitor and  measure the effectiveness of the program. The individual will work closely with Medical Informatics and Information Technology to ensure that appropriate data elements are captured and reported in a timely and accurate basis. The Director will assess, plan, implement, monitor and evaluate the Disease Management program on an ongoing basis to produce successful patient outcomes. The Director is accountable for utilizing and coordinating System and Community resources to promote program quality and produce cost-effective solutions.

Requirements:  RN with a Master’s Degree in Case Management, Health Education/Health Promotion, MPH or MHA; or MPH or MHA with CHES certification; LCSW or other Master’s prepared licensed health care professional. Education:

Entry-level pharmacy degree (e.g., B.Sc, Pharm.D) from an accredited college of pharmacy

JOB DUTIES:  Politely answers, manages, and refers, as appropriate, phone calls for bed requests.  Demonstrates effectiveness in facilitating the admission of patients, both internal and external, into the organization and collaborates with individuals necessary to ensure proper patient placement.  Collects and maintains accurate documentation of all bed board activity and records the information in logs, computerized data entry and other software systems as directed.  Collects, enters and electronically distributes bi-daily patient census information.  Performs business office functions related to patient registration, reoccurring outpatient re-registration, data collection assignments, and patient status transfers.  Performs other duties as assigned and eagerly cross trains to other departmental tasks as directed.

EXPERIENCE REQUIREMENTS:  2 to 5 years experience in customer service required. Previous experience in insurance preferred.

EDUCATIONAL REQUIREMENTS:  High school diploma or GED preferred.

SPECIAL SKILL, LICENSE AND KNOWLEDGE REQUIREMENTS:  Proficiency in typing 40 words per minute, word processing, data processing and spreadsheet utilization required.  Excellent organizational, communication and public relations skills required.

2. Performs special stains, including controls, logs in results of controls.
3. Must have the experience (minimun one year on the job) to troubleshoot/resolve processor problems.

10. Demonstrates knowledge of the occurence reporting system and uses the system to report potential safety
issues.
11. Is willing and able to be available (on-call) during the weekend (Friday night through Monday morning) to
come into the hospital to fix or repair any insturment malfunction in the Histology department. Must have
at least one full year experience on the job before being eligible for on-call.
