2 Scheduling Options:

Traditional: Full time, Mon-Fri, with weekend and holiday rotations, 8a-4:30p
OR
Weekend: Fri-Mon, 10-hour days (4 days p/week) w/company car, lower productivity, GPS…! 8a-6:30p

JOB SUMMARY: Home Health Case Manager responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Assists with directing and coordinating nursing care for patients based on established clinical nursing practice. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Supports the organization vision and mission. Uses knowledge of patient’s age and cultural diversity into the provision of patient care. Contributes to the provision of quality nursing care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care.

Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Home Health Team, and are expected in this role. Duties will include covering weekends and team member PTO, requested office tasks, and traveling outside normal work area regions to meet the needs of our patient caseload and the needs of the organization.

Job Requirements:
JOB REQUIREMENTS:
Education: Graduate of an accredited school of professional nursing
Experience: Minimum one year acute care experience qualified individuals will successfully complete the competency assessment.
Training:
License: Currently licensed or eligible for RN licensure in the State of Florida. Current CPR Certification Required.
Other: Communication and interpersonal skills, basic computer skills, and customer service skills required

Risk Manager:

The Risk Manager plans, organizes and coordinates all aspects of the Clinical Risk Management program within the facility and communicates with the Corporate Patient Safety Department. This person bears overall responsibility for developing and maintaining a comprehensive risk management and patient safety program including clinical risk identification, risk evaluation and coordination of corrective action implementation related to identified risk issues. The person also requires thorough knowledge of regulatory/accrediting agencies, statutes and reporting requirements as well as claims management reporting requirements. Current Fl Registered Nurse License

Current ACLS certification

Evaluates referred patients for swallowing and communication disorders using standard tests and videofluoroscopy. Recommends and establishes treatment programs. Provides treatment and therapy to patients with speech, language, and cleft lip and palate disorders. Prepares reports as required. Coordinates care with school and community agencies as needed. Participates in QA & I and works as a member of the interdisciplinary team. Performs other duties as assigned. EDUCATION, TRAINING, AND EXPERIENCE; CERTIFICATION/LICENSURE: Must be certified and licensed to practice in Floirida Two years experience of clinical experience in a hospital setting is required including experience with cleft lip/palate disorders

Position Requirements

Licensure / Certification : BLS, (ACLS within first year of hire)
(TNCC and PALS or ENPC within two years of hire).

Under general direction the Supervisor, performs patient care after receiving general instruction. Responsible for polysomnographic testing. Other responsibilities include patient hookup, equipment calibration, responding to patients’ needs, troubleshooting equipment; may also include performing clerical duties such as scheduling and pulling patient records. On the job training may be provided to applicants with no Sleep Lab experience who possess CRT or RRT.

Education: High School Diploma or equivalent required. Graduate of an AMA approved Respiratory Therapy school and CRT or RRT preferred.
Knowledge/Professional Licensure/Registration/Certifications: Good interpersonal skills required. Comfortable working with complex equipment. Current CPR required.
Experience: Prior experience in polysomnographic technology preferred but not required
