Documents results of evaluation, formulates treatment plan, and identifies strengths, weakness and rehabilitation problems per departmental policies as evidenced by documentation in client chart.
Consistently formulates functional, measurable goals based upon evaluation results and client/family input as evidenced by documentation in client’s chart.
Provides appropriate therapy to clients in accordance with evaluation results/recommendations as evidenced by supervisory observation and documentation in client chart.
Demonstrates knowledge of developmental stages when planning and implementing care for the person served.
Provides legible, concise and accurate documentation for patient care. Reports 100% of the time as evidenced by chart review.
Submits patient care reports, vouchers, time sheets, reimbursement reports and day off requests according to established department standards within established time frame.
Serves as contact person for family, referral source, funding source and other outside agencies.
Assures that discharge planning and arrangements for necessary follow up services are completed per department standards within established time frames.
Initiates interdisciplinary referrals when appropriate as determined by supervisory review of reports and feedback from other disciplines and utilization review.
Delegates and oversees patient treatment care plans for support staff (i.e. COTA, rehab tech, exercise physiologist, aides).
Evaluates and communicates findings and care plan.
Monitors that treatment plan is carried out per evaluation and notes are reviewed and co-signed regularly.
Effectively communicates with COTA and follows through with requests for change in treatment plan, re-evaluation of status and completion of discharge reports and notes.
Requirements: Current licensure in occupational therapy or demonstrated eligibility; Graduate of accredited occupational therapy program or equivalent and B.S. degree
