JOB SUMMARY: Completes psychosocial assessments, treatment plans and ongoing therapy in cooperation with patient and multidisciplinary team. Consults and coordinates care of patient with physician, therapists and community agencies. Delivers behavioral health care in accordance with patient evaluation, treatment plan and within the scope of their license. Provides crisis assessment and intervention. Maintains pertinent patient data in medical record in accordance with state, JCAHO and Health System guidelines. Offers in-service education programs and functions as a resource to other staff and the community as needed. Participates in development and selection of patient education materials and services. Participates in performance improvement activities. Performs other duties as assigned. Complies with all Health System, State and JCAHO policies and regulations.
ESSENTIAL DUTIES AND RESPONSIBILITIES: Investigates case situations and presents information to Psychiatrist and other members of health team, on patient’s family and social background pertinent to diagnosis and treatment. Helps patients to respond constructively to treatment and assist in adjustment leading to and following discharge. Serves as link between patient, psychiatric agency, and community. Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist. Refers patient or patient’s family to other community resources.
