Clinical Pharmacist
Job Requirements
Reviews and evaluates patients’ drug/medical history to make recommendations that optimize drug therapy by providing drug info
Conducts basic drug utilitization evaluations which include collecting, analyzing, trending, collating, and evaluating data to e synthesized into reports
Promotes interdisciplinary teamwork by working on committees, task forces, and improvement projects/initiatives
Provides education to pharmacists, students and healthcare providers through inservices or lectures and publications
Educates, assesses regimes and monitors patients in warfarin clinic, including updating treatment guidelines, policies/procedures, and performance improvement plans
Preferred Skills/Experience
Pharm.D. Degree or BS Degree with equivalent experience required. Completion on an ASHP accredit4ed residency program or equivalent experience required
Must be licensed to practice Pharmacy in the state of FL or be eligible for licensure.
