Description
Responsible for the development and the implementation of the case management process. Promotes quality patient care outcomes while supporting appropriate utilization/resource management along the continuum of care. Facilitates the multi-disciplinary case management process by coordinating the development and implementation of tools and systems for effective, efficient case management.
Qualifications
RN - BSN or equivalent Case Management/Utilization Management Exp. Supervisory Exp.-RN- BSN or equivalent Case Management/Utilization Management Exp. Supervisory Exp. Creative & Innovative Self=starter
