The role of the Case Manager is to cordinate resources and services to all patients and to promote quality and continuity of care as well as appropriate utilization of resources. The Case Manager will work in conjunction with nursing, physicians, and ancillary hospital departments and services to assess the acute and post-hospital needs of all patients, infant through geriatric. The Case Manager will coordiante the delivery of services which will allow the patient to function at an optimum level upon discharge. The Case Manager shall demonstrate knowledge of social and physical factors that affect functional status at dischare, and knowledge of community resources to meet post discharge clinical and social needs. The Case Manager shall demonstrate knowledge of the principles of growth and development over the life span and possess the abiltiy to assess data reflective of the patent’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his/her age-specific needs, and to provide the care needed as described in the deparmtent’s policies and procedures.
