Job Summary: Organizes and administers individualized professional patient care using the nursing process, is responsible for delivering care within the frame-work of standard of care and practice objectives, policies and procedures of Mercy Assisted Care. Familiar with and delivers care in accordance with Mercy Assisted Care Mission, Vision and Values. Supervises/directs patient care provided by supporting nursing care service providers. Flexibility in scheduling is required. Completes nursing assessment/reassessment of patient and documents appropriately into patient record. Collaborates with physicians and other health team members in developing a plan of care based on the assessment of patients. Initiates, manages, and participates in discharge planning process. Provides, delegates and coordinates the plan of care. Evaluates patient’s response to plan of care and updates plan of care. Responsible for providing and facilitating patient/family teaching as appropriate. Responsible for providing and/or facilitating physiological, psychosocial and spiritual care of the patient. Maintains professional standards by participating in core competencies, department meetings and on-going continuing education. Completes paperwork accurately and in a timely manner according to agency policy. Performs other duties as assigned. Education, Training and Experience: Certification/Licensure: Graduate of an accredited school of nursing.
