A. Licensure/Certification/Registration: Current Texas Registered Pharmacist certificate.
B. Education: Minimum BS Pharmacy form an accredited College of Pharmacy. The presentation of an annual licensure certificate the Oklahoma State Board of Pharmacy will be proof of compliance of the minimum requirement of 15 hours of approved continuing education each year.
C. Experience: One or more years experience as a hospital pharmacist. Previous supervisory experience preferred.
D. Special qualifications: Current Preceptor Pharmacist Certificate, computer skills, supervisory skills.
